Diathesis

Diathesis is a congenital condition of the body, characterized by a special predisposition to certain diseases. Diathesis is not yet a disease or pathological condition. When diathesis breaks the balance of functions and metabolic processes of the organism, its reactivity changes and adapts to the external environment. In diathesis, even ordinary stimuli cause a pathological reaction in the body and manifest as a disease. The latent period can last indefinitely; diathesis can be revealed quite unexpectedly due to changes in the external conditions.

Three forms of diathesis are most common: in infants of the first years of life - exudative-catarrhal (see Exudative diathesis ), in preschool children - lymphatic-hypoplastic diathesis (see), in children of school age - neuro-arthritic. Some consider these forms as variants of a single diathesis, varying with age, but most consider them as independent states. A peculiar form is allergic diathesis, which unites a number of forms of increased sensitivity of the body to allergens (food, infectious, etc.).

Prevention consists in changing the environment, nutrition, life, hardening of the body, increasing its resistance. The treatment of diathesis and diseases that have arisen on the background of diathesis, is currently carried out by allergists in specialized departments according to the general rules for the corresponding disease, taking into account the characteristics of the existing form of diathesis.

Diathesis, or childish pruritus , is a fairly common chronically flowing recurrent dermatosis . Prurigo may occur in an infant or somewhat later in the 2nd-3rd year of life.

A characteristic clinical sign of this disease is severe itching of the skin, followed by a rash of blisters and serous lenticular nodules of red or pink color, with tiny bubbles filled with a transparent liquid on their surface. With suppuration (most often due to scratching) of the elements, pustules or even large boils can occur. The rash may be accompanied by a small fever, their development occurs seizures that last from several weeks to several months.

First of all, the skin of the face, especially the forehead, cheeks, chin, nose wings, scalp. Often the rash is localized on the extensor surfaces of the upper and lower extremities. Sometimes there is an increase in the femoral and inguinal lymph nodes. The skin on the affected areas thickens, loses its elasticity, becomes rough. An important condition for recovery of the patient - the identification of the allergen. However, the allergen can be recognized only when the disease lasts a long time (up to several months, even years).

Treatment . Commonly used corticosteroid ointments (prednisolone, fluorocort, dermozolon, depersolone, sinalar, flucinar). These drugs in most cases bring a therapeutic effect, but often it is short. Corticosteroid ointments can be used for no more than 2-3 months, after which it is necessary to switch to other drugs. Useful 1-2-2% boron-bismuth ointment on naphthalan or the same concentration of bismuth-tar ointment. Pre-clean with pure alcohol surrounding healthy skin, then apply a thin layer of ointment on the rash. So, where possible (for example, on the limbs), it is better to rub the ointment and apply a gauze bandage.

Inside, vitamin A (concentrate) is administered at a dose of 1 to 5 drops, depending on the age, 3 times a day. For a short time (3-4 weeks), diphenhydramine and other antihistamines may be used. They put lotions from gularny water on moist areas: take 10 layers of gauze, moisten with this water, squeeze and apply on the affected area (30-40 minutes 4-5 times a day).

Many medical workers with pruritus completely cancel baths and showers. However, this is not correct, since there are a large number of pathogenic microorganisms on the skin of patients with scratching or other eruptions that must be removed. Cleaning baths and showers with baby soap should be carried out 1 time in 7-10 days.

The diet of children suffering from pruritus should not contain foods that can serve as a food allergen (chocolate, honey, cocoa , caviar, citrus fruits, strawberries). It should be remembered that the same product causes allergies in one child, and not in another.

With the observance of diet and other preventive measures diathesis in children in most cases can be interrupted, and full recovery occurs.
